General Description of the Job Class

The Duke Primary Care Population Health Virtual Pharmacist will provide innovative pharmacy services to patients served by Duke Primary Care (DPC). Specifically, the clinical pharmacist will function as a Clinical Pharmacist Practitioner (CPP) for select DPC clinic(s) by conducting clinical services virtually. The clinical pharmacist shall serve as an educational resource, providing pharmacy expertise for DPC healthcare providers and staff. The position may precept pharmacy students and residents.

Duties and Responsibilities of this Level

Telehealth Clinical Pharmacist Responsibilities

Clinical Practice:

  • Participates in a virtual clinic, care management activities, and provides clinical pharmacy services and oversight for safe use of medication. This includes prescribing and monitoring of medications within the scope of practice designed collaboratively with physician colleagues. This may include, but is not limited to:
    • Completes medication reconciliation, and screens for drug interactions, therapeutic duplications, omissions, etc.
    • Provides medication counseling
    • Prescribes and adjusts medications as permitted through collaborative practice agreement
    • Performs patient assessment for medication-related factors and orders laboratory tests necessary for monitoring therapeutic outcomes and/or adverse response of medication therapy
    • Coordinates patient care with other outpatient or inpatient teams and participates in multidisciplinary reviews of patients' progress, when applicable.
    • Documents the care provided in patients' records
    • Serves as resource to DPC providers, staff, and other healthcare professionals to provide medication reviews.
    • Assists in identifying and developing interventions for DPC patients that are not meeting target quality measures
    • Serves as resource to DPC network providers and Population Health Management Office (PHMO) staff to assist in addressing medication access and medication adherence challenges
    • Serves as a resource to DPC providers and staff for timely general drug information and payer pharmacy policy
    • Provides educational in-services on medication-related topics to patients, DPC providers, DPC staff, and other healthcare professionals
    • Ensures continuity of care and may support transitions of care for select DPC patients
    • Provides virtual services to DPC patients as determined based on oversight from DPC practice medical director and/or other DPC leadership, as well as pharmacy leadership
    • Interprets data related to medication safety and effectiveness and provides support to improve safety and reduce risk to patients. Reports events and/or concerns via the Duke Health Safety Reporting System (SRS)
    • Provides oversight of cost-effective use of medications and treatments. This includes risk reduction and efficient utilization of resources to optimize care
    • Participates in developing guidelines, algorithms, and care pathways to improve the care and safety of patients
    • Participates in efforts to maintain TJC accreditation standards within the realm of medication use at clinical practice sites
    • Facilitates prescription fulfillment through coordination with retail pharmacies
    • Develops and participates in pharmacy or medication-related research projects and quality improvement initiatives
    • Educates medical, nursing, and pharmacy staff about the management, prescribing, and monitoring of drug therapies

Administrative Practice:

  • Participates in DPC and other Duke Health committees and initiatives related to primary care and pharmacy practice
  • Coordinates with others to maintain up-to-date knowledge of pharmacy benefit design and access in support of pharmacy practice
  • Maintains familiarity with pharmacy programs across Duke Health and functions as an integral member of the pharmacy team
  • Models behaviors that exemplify Duke Health values, including teamwork, integrity, diversity, excellence and safety
  • Maintains outstanding skills as a clinical pharmacy practitioner and consistently demonstrates the value of pharmacy services

Minimum Qualifications

Education

  • Doctor of Pharmacy Degree OR BS Pharmacy Degree

Experience

  • With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.
  • With Doctor of Pharmacy degree, no experience is required.
  • 2+ years of clinical experience preferred
  • PGY2 Ambulatory Care residency preferred
  • PGY1 or Pharmacy Practice Residency preferred

Degrees, Licensures, Certifications

  • Licensed Pharmacist or eligible to practice pharmacy in North Carolina
