Job Brief

We are seeking a dedicated Virtual Clinical Pharmacist to join our healthcare team serving employer based advanced primary care clinics.

You will be responsible for providing clinical pharmacy services and implementing pharmacy care plans for patients in outpatient settings.

The ideal candidate will have a strong understanding of pharmacotherapy, direct patient care, understand how to collaborate with other clinical team members (nurses, medical assistants, and medical providers) and be able to thrive in a fast-paced environment.

The Clinical Pharmacist plays a vital role in educating patients about medication usage, potential side effects, overall healthcare management, and working closely with clinic staff to optimize pharmacy related tasks such as prior authorizations.

Responsibilities

Provide comprehensive medication management services including assessment, pharmacotherapy evaluation, and intervention and education.

Identify and outreach to the top 15% of high cost/risk clinic patients to evaluate for pharmacy saving opportunities.

Be available on live video stream onto a device physically located at the clinic for provider or patient consults, clinic staff questions, and patient appointments.

Develop, implement, and manage pharmaceutical care plans for primary care clinic patients.

Participate in patient counseling, providing medication-related health education to enhance patient understanding.

Monitor patient responses and therapeutic outcomes on an ongoing basis.

Collaborate with healthcare professionals to ensure optimal patient care.

Conduct medication reconciliation and identify and resolve medication-related problems.

Stay updated with latest therapeutic advancements, drug approval processes, and regulations.

Maintain accurate patient medication records and document pharmacy interventions into the population health management tool.
