Job summary

Responsibilities

• Provides medication management services in a collaborative manner.
• Reviews patient medication history, educates patients on drug therapy, establishes outcomes, develops plans and monitors drug interactions.
• Maintains and updates patient profiles with demographics, diagnosis, allergies and current medications.
• Monitors drug therapy for appropriateness, dosage, form, regimen and interactions.
• Provides drug information to healthcare professionals and patients; documents interventions.
• Counsels patients on discharge medications and verifies medication orders.
• Performs medication order entry when needed; discusses order clarifications with prescribers.
• Facilitates discharge planning and smooth transitions to alternate care sites.
