Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy (BPharm).
• Residency: Pharmacy Practice - General (Preferred).
• 1-2 years of previous job-related experience.
• ACLS (Advanced Cardio Life Support) certification preferred.

Responsibilities

• Provide pharmaceutical care, oversight, monitoring, preparation, dispensing and documentation of patient medication therapy; coordinate with health care team to ensure compliant practice.
