Job summary
Premise Health is seeking a Full Time Collaborative Practice Pharmacist to provide care to client employees at our Health Center in Whitakers, NC. The pharmacist will perform medication management, reconciliation, education, preventive care, and collaborative care under state regulations to improve patient outcomes and provider productivity.
Requirements include a Pharm D or Bachelor of Pharmacy Degree, current licensure, and certifications such as Certified Diabetic Educator or coaching
Qualifications
- 3+ years of disease state management or clinical pharmacy experience preferred.
- Licensure as a pharmacist in good standing.
- Experience communicating with diverse client populations.
Responsibilities
- Provide direct patient care to improve outcomes and provider productivity.
- Conduct medication management including history intake, reviews, and adjustments.
- Perform medication reconciliation to prevent errors and ensure adherence.
- Deliver patient education and wellness coaching to support behavior change.
