Job summary

Cape Fear Valley Health System in Fayetteville, NC seeks a Clinical Pharmacist Practitioner to lead medication therapy and education across ambulatory settings. You will collaborate with physicians and nurses to optimize patient outcomes and support system-wide changes.

The role includes precepting residents and students, vaccinations, and contributing to quality improvement initiatives within the Diabetes and Endocrine Center. Licensure in NC and CPP eligibility within 12 months are required.

Qualifications

• PharmD degree and NC license required.
• CPP license within 1 year of hire preferred.
• Residency training (PGY1) preferred; PGY2 ambulatory care preferred.
• Board certification (BCPS) or equivalent preferred.

Responsibilities

• Develop training programs to support system changes.
• Provide medication therapy management, symptom management, and chemotherapy/medication education per protocol.
• Serve as a pharmacy resource with literature searches and drug information inquiries.
• Maintain professional relationships with CFVHS staff.
• Participate in organizational improvement activities.
• Complete all required documentation.
• Precept PGY1 residents.
• Precept PharmD students.
• Hold self and staff accountable for patient service.
• Develop and implement quality improvement measures.
