Job summary

Premise Health is seeking a Full Time Collaborative Practice Pharmacist to provide care at our Health Center in Columbus, IN. The role focuses on direct patient care, medication management, and preventive services within a collaborative practice framework.

You will work with client employees, perform education and behavioral counseling, and contribute to improved outcomes and productivity. This position includes comprehensive benefits and professional growth opportunities.

Qualifications

• Pharm D or Bachelor of Pharmacy Degree required.
• Current and unrestricted pharmacist licensure.
• Certified Diabetic Educator certificate or willingness to obtain within one year of hire date.
• Wellcoaches coaching certificate, a coaching certificate from an ICF-accredited program, or equivalent advanced training in Motivation and Leading change or willingness to obtain.

Responsibilities

• Provide direct patient care and improve provider productivity.
• Medication management including histories, reviews, and monitoring.
• Medication reconciliation on changes to avoid errors.
• Education and behavioral counseling for patient-specific goals.
• Preventive care services and labs per protocol.
• Collaborative Care Practice Agreements to define responsibilities.
• Serve as resident pharmacologic expert for staff providers.
• May perform other duties as assigned.
