Job summary

Aurora Health Care in Wisconsin seeks a Pharmacist to ensure safe, effective medication use across the patient care continuum. You will review orders, dispense and compound medications, and uphold formulary adherence while prioritizing patient safety.

Responsibilities include enabling continuity of care at admission, transfer and discharge, training new staff, and collaborating with providers to optimize therapy. Bachelor's in Pharmacy and license required; no experience needed.

Qualifications

• Pharmacist license issued by the state in which the team member practices.
• Bachelor's Degree in Pharmacy.
• Ability to stand for long periods of time.
• Will occasionally lift up to 35 lbs.
• Must have functional speech, vision, and hearing.

Responsibilities

• Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems.
• Facilitates medication-related continuity of care at admission, transfer and discharge.
• Reviews and accurately interprets medication orders.
• Develops competency in dispensing and compounding medications consistent with good pharmaceutical practice including compounding technique, packaging, and labeling.
• Practices in compliance with department policies & procedures, as well as local, state and federal regulations.
• Ensures formulary adherence, recommends cost effective therapies to providers, and minimizes waste in daily practice.
• Actively supports procedures and initiatives to prevent medication events. Reports events through appropriate channels.
• Orients/trains new pharmacy team members. May work with interns, residents and students to guide daily activities. Provides direction to pharmacy technicians to ensure smooth workflow and service standards.
