Qualifications

• Doctor of Pharmacy Degree (PharmD) required and licensed to practice in Texas.
• 1–2 years of residency training and/or at least two years of clinical pharmacy work experience.
• Pharmacist License with the Texas State Board of Pharmacy (TSBP) in good standing; Board Certification (BPS) preferred.
• Ability to prioritize, make critical decisions, respond quickly to emergencies, and communicate effectively across hospital disciplines.
• Knowledge of state/federal laws and hospital policies; commitment to interdisciplinary teamwork and patient safety.

Responsabilités

• Provide pharmacotherapy support to medical staff to optimize medication selection, dosing, and monitoring.
• Participate in multidisciplinary rounds and collaborations to optimize pharmacotherapeutic care.
• Evaluate the appropriateness and effectiveness of the patient’s medications; serve as an evidence-based information source.
• Contribute to antimicrobial stewardship and education of nursing, medical staff, and pharmacy staff.
