Job summary

TriStar StoneCrest Medical Center in Smyrna, Tennessee, is seeking a Clinical Pharmacist to support safe medication use and patient care across shifts.

You will fill orders, monitor therapies, provide drug information, and collaborate with physicians and nurses. The role emphasizes compliance with hospital policies, patient safety, and opportunities for professional growth within a growing health system.

Qualifications

  • Graduate of an Accredited School of Pharmacy.
  • Licensure in the State of TN; Board Certification preferred.
  • One-year hospital experience preferred; Residency preferred.

Responsibilities

  • Assist in daily pharmacy tasks including filling orders and preparing medications.
  • Monitor patient therapeutic medications and provide drug information.
  • Check medications and IV mixtures; assist with compounding as needed.
  • Consult with physicians and nursing staff regarding patient reactions and errors.
  • Identify potential drug interactions and contribute to quality improvement.
  • Maintain documentation in computerized systems and adhere to policies.
  • Apply age-specific knowledge to patient care responsibilities.

Clinical Pharmacist

The Clinical Pharmacist is responsible for assists in preforming daily pharmacy tasks, including but not limited to: filling orders for medications, monitoring patient therapeutic medications, providing drug information, checking medications, IV mixtures and compounding medications, prior to distribution.

What you will do in this role:
  • You will utilize good problem solving abilities, making sure that problems are resolved, calling on other knowledgeable parties as necessary.
  • You will maintain strong knowledge of hospital policies and government regulations and dispense accordingly.
  • You will consult with physicians and nursing staff regarding patient reactions, errors and complaints.
  • You will determine and consult on possible drug interactions.
  • You will assist with quality improvement initiatives, inventory and ordering of pharmaceuticals.
  • You will monitor, record and communicate as appropriate utilizing computerized documentation systems.
  • You will utilize knowledge of age specific needs of patient in performance of duties and responsibilities.
