Job summary

Tower Health's Reading Hospital in Reading, PA is seeking a Clinical Pharmacist to validate medication orders, compound medications, and provide drug information to clinicians and patients.

You will support patient care with teamwork across pharmacy leadership and hospital departments, while adhering to protocols and maintaining documentation of clinical activities and outcomes. A PA Pharmacist License is required.

Qualifications

  • Doctorate Degree or combination of relevant education & experience may be considered in lieu of Doctorate Degree.
  • PA Pharmacist License required.
  • Clinical Pharmacist Experience in Healthcare Setting preferred.

Responsibilities

  • Validates accuracy and appropriateness of medication orders.
  • Compounds medications and provides prescribed medications accurately.
  • Provides consultative services to internal and external stakeholders regarding medications and interactions.
  • Follows medication-related protocols, policies and procedures.
  • Documents clinical activities and outcomes.
  • Maintains relationships with leadership, staff and other hospital departments.

Available Pharmacist Opportunities:

  • Evening Position: 12:30PM-9PM; Every 3rd Weekend & Holiday Rotation
  • 50/50 Position: 50% 1st Shift, 50% 2nd Shift - Every 3rd Weekend & Holiday Rotation
  • 7 On / 7 Off Position: M-F 12:30PM – 11PM & Sat/Sun 8:30AM-9PM for one week. Off second week. Holidays as they fall in 7 On / 7 Off schedule
The Clinical Pharmacist
  • Validates accuracy and appropriateness of medication orders.
  • Compounds medications and accurately provides prescribed medications according to professional standards, practices and specific indications or orders.
  • Provides consultative services to internal and external system constituents regarding medications, interactions, and other drug related information.
  • Follows defined medication related protocols, policies and procedures.
  • Documents clinical activities and applicable outcomes.
  • Achieves and maintains satisfactory working relationships with pharmacy leadership, pharmacy staff and other hospital departments, committees and administrators.
  • Places a strong emphasis on patient care with the patient as the highest priority and central focus in all endeavors and undertakings of the individual and the department.
