Protouch Staffing seeks a Clinical / Long-Term Care Pharmacist for a closed-door LTC pharmacy in Eastlake, OH. You will work with prescribers, nurses, and facility staff to ensure residents receive safe, effective, and timely medication therapy.

Qualifications

  • Active and unrestricted Pharmacist license required.
  • Bachelor's degree in Pharmacy or PharmD credentials welcome.
  • Prior long-term care pharmacy experience preferred.
  • Experience with parenteral admixtures/IV preparations is a plus.
  • Strong knowledge of pharmacology, physiology, medical terminology, and medication therapy.
  • Excellent clinical judgment and attention to detail.
  • Strong communication and collaboration skills.
  • Ability to work effectively with nurses, prescribers, and LTC facility staff.
  • Experience with pharmacy inventory management is preferred.

Responsibilities

  • Provide comprehensive pharmaceutical care to residents in long-term care settings.
  • Review and verify medication orders for accuracy, appropriateness, interactions, and potential drug-related problems.
  • Conduct medication regimen reviews and identify opportunities to optimize therapy.
  • Make appropriate clinical interventions, including dosage recommendations, monitoring, and therapy adjustments.
  • Communicate and collaborate with physicians, nurses, prescribers, and facility staff regarding patient medication needs.
  • Monitor medication therapy and support continuity of care for LTC residents.
  • Accurately dispense and verify medications while maintaining compliance with applicable pharmacy regulations and facility standards.
  • Maintain accurate medication records, documentation, and required pharmacy records.
  • Manage pharmacy inventory, including medications and supplies, to ensure timely availability.
  • Educate facility staff regarding medication changes, administration protocols, medication safety, and appropriate medication use.
  • Contribute to overall pharmacy quality, safety, and operational efficiency.

Schedule: Monday-Friday + Rotational Saturdays | 8:00 AM-5:00 PM

Position Overview

We are seeking a Clinical / Long-Term Care Pharmacist to join a growing closed-door long-term care pharmacy serving nursing-home communities.

This is an excellent opportunity for a pharmacist who is passionate about medication safety, clinical pharmacy services, and patient-centered care. You will work closely with prescribers, nurses, and long-term care facility staff to ensure residents receive safe, effective, and timely medication therapy.

The closed-door setting provides a focused environment without walk-in outpatient or traditional retail traffic.

Key Responsibilities
