Job summary

AdventHealth Palm Coast is seeking a Clinical Performance Nurse (CPN) in Palm Coast, FL to enhance patient care through quality improvement and care coordination, without direct bedside duties.

You will collaborate with hospital teams, analyze clinical data, and drive strategies for discharge readiness, patient satisfaction, and care transitions, with a 40-hour week, Monday–Friday, no nights or weekends.

Qualifications

• Minimum of 2 years of acute care nursing experience.
• Active RN licensure in good standing; BSN preferred.
• Proficient with electronic medical records and Microsoft Office.
• Experience with quality improvement initiatives.
• Experience in case management, utilization review, or post-acute services is helpful but not required.
• Authorized to work in the United States.

Responsibilities

• Demonstrate strong clinical knowledge in an acute care setting.
• Assess patients' clinical status and discharge readiness.
• Ensure effective communication across care team.
• Focus on improving overall quality of patient care.
• Participate in daily workflows and advocate for patient goals in the care plan.
