Job summary

Sound Physicians in Adrian, MI is seeking a Clinical Performance Registered Nurse (CPN) to support quality improvement and patient care without direct bedside nursing. The role emphasizes collaboration with hospital teams, discharge planning, and program metrics to enhance outcomes.

The position offers a 40-hour work week, Monday–Friday, with no nights or weekends and includes paid holidays and opportunities for bonus.

Qualifications

• Minimum of 2 years of acute care nursing experience.
• Active RN licensure in good standing; BSN preferred.
• Proficient in technology, EMR systems, and Microsoft Office.
• Experience with quality improvement initiatives to enhance patient satisfaction.
• Experience in case management, quality, utilization review, transitional care, or post-acute services is helpful but not mandatory.

Responsibilities

• Demonstrate strong clinical knowledge in an acute care setting.
• Assess patients' clinical status and readiness for discharge.
• Ensure effective communication across the care team as patients progress.
• Focus on improving the overall quality of patient care.
• Participate in daily workflows and advocate for patient goals in the care plan.
