Job summary

LifeBridge Health is seeking a Clinical Performance Coordinator to support primary care practices in advancing high-quality, value-based care delivery. You will engage patients, coordinate care, and drive quality improvement across population health initiatives.

You will work with care teams to close gaps in care, promote AWVs, and track KPIs using EHR data, improving transitions of care and preventive services.

Qualifications

• 2–4 years in healthcare operations, population health, care coordination, medical office workflows, quality improvement.
• Experience with patient engagement and chronic disease management.
• Ability to work on-site in the Baltimore area.

Responsibilities

• Identify and outreach to attributed and unaligned patients to engage with primary care providers.
• Coordinate follow-up after hospitalizations and ED visits to support continuity of care.
• Utilize payer reports, patient registries, and EHR tools to identify gaps in care and support quality initiatives.
• Monitor KPIs, dashboards, and population health metrics to report progress.
• Support AWV outreach and scheduling to improve preventive care.
