Job summary

Thousand Branches Wellness is seeking a Clinical Outreach Liaison to lead clinically-informed outreach in Mesa, AZ. The position emphasizes field-based, externally focused work with flexibility for evening or weekend events to support community presence and outreach goals.

Qualifications

• Master’s degree in a behavioral health field preferred.
• Active licensure or registration strongly preferred.
• 1–3+ years of experience in behavioral health (clinical, case management, or utilization review experience preferred).
• Strong understanding of behavioral health diagnoses and levels of care (IOP/PHP/residential).
• Established relationships within the behavioral health community.
• Experience with CRM or referral tracking systems.

Responsibilities

• Execute assigned outreach plans with a focus on clinically appropriate referral development.
• Build and maintain relationships with referral partners, including therapists, psychiatrists, hospitals, and community providers.
• Conduct regular in-person outreach, meetings, and follow-ups to maintain strong community presence.
• Collaborate with Admissions and clinical teams to support appropriate and timely referral transitions.
• Track outreach activity and referral trends to support consistency and follow-through.

Through clinically informed community outreach and referral engagement. This role is responsible for the execution of outreach activities with a strong clinical lens, ensuring referral partners are supported in identifying appropriate level-of-care placements. Working within established outreach priorities, the Liaison will serve as a clinical resource in the community, helping bridge gaps between providers and treatment services.

This is a field-based, externally focused role and does not include responsibility for internal program operations, staffing, or clinical service delivery. This position may entail evening and/or weekend hours as needed for community events.
