Thousand Branches Wellness - Minnesota is seeking a Clinical Outreach Liaison to lead field-based outreach for clinically appropriate referrals. Engage with therapists, hospitals, and community providers, and coordinate with Admissions and clinical teams to ensure timely transitions.

The role emphasizes relationship-building, clinical guidance for referral partners, and consistent execution of outreach plans, with potential evening/weekend events as needed.

Qualifications

• Bachelor's degree required.
• Master’s degree in behavioral health preferred.
• Active licensure or registration strongly preferred.
• 1–3+ years of experience in behavioral health (clinical, case management, or utilization review experience preferred).
• Strong understanding of behavioral health diagnoses and levels of care (IOP/PHP/residential).
• Established relationships within the behavioral health community.
• Experience with CRM or referral tracking systems.

Through clinically informed community outreach and referral engagement. This role is responsible for the execution of outreach activities with a strong clinical lens, ensuring referral partners are supported in identifying appropriate level-of-care placements. Working within established outreach priorities, the Liaison will serve as a clinical resource in the community, helping bridge gaps between providers and treatment services.

This is a field-based, externally focused role and does not include responsibility for internal program operations, staffing, or clinical service delivery. This position may entail evening and/or weekend hours as needed for community events.
