Qualifications

• OD degree from an accredited School of Optometry.
• California Optometrist license required at hire.
• Therapeutic Pharmaceutical Agents (TPA) required for external employees hired after November 2009.
• National Provider Identifier (NPI) required or to be obtained prior to employment start date.

Responsibilities

• Conducts and documents patient history including chief complaint, relevant ocular health, family and medical history, medications, current spectacles/contacts and data needed to manage vision and ocular health needs.
• Performs testing as needed with standard ophthalmic equipment (retinoscopy, refraction, lensometry, visual fields, slit lamp, ophthalmoscopy, gonioscopy, tear testing, Amsler grid, etc.) and documents findings.
• Interprets data from clinical exams, makes diagnoses, and documents impressions and action plans including referrals and prescriptions.
• Collaborates with optical staff and ophthalmology to ensure timely, quality service; participates in staff meetings and raises system improvements.
• Maintains all licensing and training requirements (CE, CPR, safety training, PPD).
• Duties include writing or dictating referrals and information letters to other providers, or handling school forms, DMV, FAA documents as appropriate.
