Job summary

Graceworks Lutheran Services seeks a Clinical Coordinator RN to lead the implementation and coordination of VNA Health Center’s primary care program across multiple sites. This role blends administrative leadership with direct nursing oversight of nurses, CNAs, and phlebotomists to ensure efficient clinic flow and high-quality care.

The position emphasizes policy implementation, staff training, and adherence to HIPAA and Joint Commission standards, with compensation between $86,000 and $100,000

Qualifications

• Registered Nurse in Illinois required.
• Bachelor’s degree in nursing preferred.
• Two years of nursing experience preferred.
• Supervisory experience preferred.
• Ability to function as an integral part of the management team.
• Superior communication and public speaking skills required.

Responsibilities

• Coordinate nursing, CNA and phlebotomy staff activities to meet patient needs and ensure efficient clinic operation.
• Oversee staff training, competency, performance appraisals and disciplinary actions per policy.
• Orient new staff to EMR and assist in orientation of nurse practitioners and medical staff.
• Ensure vaccine policies and procedures are followed and manage vaccination inventory.
• Collaborate with providers on continuity of client care and participate in quality improvement processes.
