AppleGate Recovery, a division of BayMark Health Services, is seeking a Full Time Clinical Coordinator to join our outpatient treatment program. You will supervise and monitor patient care, ensure accurate records, and support staff in delivering evidence-based services.

The role emphasizes leadership, adherence to federal/state regulations, and effective communication with patients and referral sources. A background in behavioral health and care coordination is preferred.

Formación

• Bachelor’s or Master’s degree in the behavioral health field highly preferred. (Additional related experience may substitute for educational component)
• Experience in care management for patients in substance abuse treatment facility is desired
• Strong communication and leadership abilities with staff and patients

Responsabilidades

• Maintain quality care and monitor patient treatment
• Ensure accurate and current clinical records
• Conduct internal program reviews and education counseling
• Assess training needs with Program Director and report them
• Ensure staff compliance with Federal and State regulations
• Provide intake/readmission services and follow-up with discharged patients
• Lead monthly staff trainings and chart monitoring
• Participate in community relations and outreach activities
