Job summary

AppleGate Recovery seeks a dedicated Care Coordinator to support individuals on their path to recovery. You will provide care management services, help build support networks, and connect patients to community resources in a compassionate outpatient setting.

The role emphasizes accurate documentation, collaboration with healthcare providers, and a passion for helping others achieve recovery goals. A CRS certification is preferred, with eligibility to obtain within 6 months.
