Job summary

HC is seeking an experienced Clinical Nutrition Manager / Registered Dietitian to lead the facility's clinical nutrition program in a Skilled Nursing & Rehabilitation setting. This full-time role focuses on exceptional nutritional care, compliance, and positive resident outcomes.

You will collaborate closely with nursing, dietary services, physicians, and the interdisciplinary team to implement care plans and monitor nutrition-related interventions.

Qualifications

• Registered Dietitian (RD) credential required.
• Active state licensure or certification where applicable.
• Experience in Skilled Nursing/Long-Term Care or healthcare setting preferred.
• Leadership or management experience preferred.
• Strong knowledge of clinical nutrition, therapeutic diets, enteral nutrition, and regulatory requirements.

Responsibilities

• Lead and oversee the facility's clinical nutrition program.
• Conduct comprehensive nutritional assessments and reassessments.
• Develop, implement, and monitor nutrition care plans.
• Monitor residents' nutritional status and dietary intake.
• Provide clinical oversight for residents with complex nutritional needs.
• Collaborate with interdisciplinary team and participate in care plan meetings.
• Ensure accurate clinical documentation and regulatory compliance.
• Support quality improvement and survey readiness initiatives.
