Job summary

Heritagespring is seeking a Registered Dietitian for a skilled nursing facility to lead clinical nutrition management and individualized food service for residents. You will report to the facility Administrator and coordinate closely with the corporate dietitian to ensure high-quality resident care.

Responsibilities

• Performs clinical assessments and updates patient care plans.
• Maintains knowledge of the PDPM system to capture patient needs.
• Documents in PointClickCare and uses dietary software for meal planning.
• Collaborates with facility chef and dietary dept to optimize food service.
• Participates in meal services, monitors resident meal intake, and trains staff.
• Maintains dietary cost control with nutrition interventions and supplements.
• Collaborates with families, physicians, and IDT for diet liberalization and self-directed care.
• Communicates nutrition orders and weight changes to physicians and admin.
• Acts as a nutrition leader, resolves dietary issues, and supports the team.
