Job summary
Manchester Center for Rehabilitation and Healing is seeking a dedicated Clinical Nutrition Manager / Registered Dietitian (RD) to join our skilled nursing and rehabilitation team. You will oversee the clinical nutrition program and collaborate with an interdisciplinary team to provide resident-centered care.
The ideal candidate will have a Bachelor's in Dietetics or Nutrition and RD/RDN credentials, with experience in long-term care, and strong clinical assessment, communication, and leadership
Qualifications
- RD/RDN credential required or preferred per state/facility
- Bachelor's degree in Dietetics or Nutrition
- Experience in skilled nursing/rehabilitation/long-term care preferred
- Strong knowledge of clinical nutrition, therapeutic diets, and care planning
- Experience with MDS, care plans, and regulatory requirements in long-term care preferred
- Strong clinical assessment, communication and organizational skills
- Ability to work effectively as part of an interdisciplinary team
- Compassionate and resident-focused care
- Strong leadership and problem-solving abilities
Responsibilities
- Oversee the clinical nutrition services and ensure high-quality, resident-centered nutrition care
- Complete and oversee comprehensive nutritional assessments for residents
- Develop individualized nutrition care plans based on medical conditions, dietary needs, preferences, and goals
- Monitor residents at nutritional risk and respond to concerns
- Collaborate with Nursing, Physicians, Speech Therapy, Rehabilitation, and other interdisciplinary team members
- Participate in care plan meetings and provide clinical nutrition recommendations
- Monitor weights, meal intake, laboratory values, and other clinical indicators
- Make recommendations regarding therapeutic diets, supplements, and nutrition interventions
- Communicate with residents, families, nursing staff, and medical providers about nutrition concerns
- Supervise and support dietary/nutrition team members
- Ensure compliance with federal, state, and facility regulations and survey readiness
- Assist with quality improvement initiatives and documentation
- Provide education and training to staff on nutrition topics
