Clinical Nurse II - Diabetes Care Coordinator

Job Summary:

We are seeking a CLINICAL NURSE II in our Duke Endocrine Cary Clinic located in Cary, NC to be an integral member of the healthcare team and provide direct and indirect care to patients.

Job Duties and Responsibilities:

  • Plan, provide, and document professional nursing care in ambulatory settings, utilizing the nursing process, in accordance with physician orders and established policies and procedures, under the supervision of the nurse manager, charge nurse, or practice manager. Specific functions delegated to the Diabetes Care Coordinator include but are not limited to:
  • Instruct patients on proper administration of injectable medications (e.g., insulin, injection pens, glucagon, Baqsimi, etc.) following provider visits.
  • Assist Diabetes Educators in providing diabetes self-management education to existing patients as needed.
  • Facilitate effective care coordination of diabetes-related phone calls, emails, and My Chart messages.
  • Assist Diabetes Educators in ensuring patients are educated in the proper use of diabetes-related technologies such as insulin pumps, Continuous Glucose Monitors, and glucometers.
  • Apply diagnostic CGMs and educate patients on their application and use for ongoing personal management.
  • Work with patients to ensure enrollment into NCCare 360.
  • Assist clinical staff with connecting patient diabetes devices and ensuring reports are available based on patient disease status.
  • Support ongoing education of clinical staff regarding diabetes technology and clinical information.

Additional responsibilities include delegating tasks to UAP, participating in performance improvement activities, supporting staff development, collecting and analyzing patient data, maintaining professional competencies, preparing for daily clinic needs, and other related duties.

Job Eligibility Requirements:

  • Graduation from an accredited BSN, Associate's Degree in Nursing, or Nursing Diploma program. All registered nurses without a BSN must enroll in a BSN program within two years and complete it within seven years of start date.
  • Minimum of twelve months of clinical experience.
  • NC RN license required. If holding a license from another compact state, NC licensure must be obtained within 90 days of hire.

Job Hours:

Day shifts, Monday-Friday, with no weekends or holidays.
