Clinical Nurse II – Diabetes Care Coordinator
Job Location: 100 Duke Health Cary Place, Cary NC, 27519
Job Summary:
We are seeking a Clinical Nurse II in our Duke Endocrine Cary Clinic located in Cary, NC to be an integral member of the healthcare team and provide direct and indirect care to patients.
Job Duties and Responsibilities:
- Plan, provide, and document professional nursing care in ambulatory settings, utilizing the nursing process, in accordance with physician orders and established policies and procedures, under the supervision of the nurse manager, charge nurse, or practice manager. Specific functions delegated to the Diabetes Care Coordinator include but are not limited to:
- Instruct patients on proper administration of injectable medications (e.g., insulin, injection pens, glucagon, Baqsimi, etc.) as needed following provider visits.
- Assist Diabetes Educators in providing diabetes self-management education to existing patients as needed.
- Facilitate effective care coordination of diabetes-related phone calls, emails, MyChart messages.
- Assist Diabetes Educators to ensure patients are educated in the proper use of diabetes-related technologies such as insulin pumps, Continuous Glucose Monitors, and glucometers.
- Apply diagnostic CGM and educate patients in application and use of prescribed CGMs for ongoing personal use.
- Work with patients to ensure they are enrolled into NCCare 360.
- Assist clinical staff with connecting patient diabetes devices and ensuring appropriate reports are available based on patient disease status.
- Assist with ongoing education of clinical staff with diabetes technology and clinical information.
- Delegate tasks to UAP.
- Participate in clinic or entity-level performance improvement activities.
- Support the development of other staff and formal learners. Participate in identifying clinical, operational, and safety-related performance improvement opportunities.
- Collect and analyze patient data, make recommendations for nursing care plans, evaluate nursing care provided, document and communicate nursing actions taken and patient responses.
- Participate in own professional development by maintaining required competencies, identifying learning needs, and seeking appropriate educational opportunities.
- Prepare for daily clinic needs.
- Perform other related duties incidental to the work described herein.
Job Eligibility Requirements:
- Graduation from an accredited BSN, Associate's Degree in Nursing, or Nursing Diploma program. All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and complete it within seven years.
- Twelve months of appropriate clinical experience is required.
- NC RN license is required. If the candidate has another compact state RN license, NC licensure must be obtained within 90 days of hire.
Job Hours:
The schedule for this position is day shifts, Monday-Friday, with no weekends or holidays.
