Job summary
The Department of Developmental Services (DDS) - South Region in Connecticut is seeking a Clinical Nurse Coordinator for a full-time, first shift role in Wallingford. The position requires RN licensure in CT or a multistate/Compact license, and travel between DDS locations within the state may be needed.
A hybrid work arrangement may be requested after the probation period. Ideal candidates have 3+ years of professional nursing experience and experience coordinating clinical programs with
Qualifications
- Three (3) years of professional nursing experience.
- A Bachelor's degree in nursing may substitute for one year of general experience.
- A Master's degree in nursing may substitute for one additional year of general experience.
- Experience with documentation and reporting in clinical settings.
Responsibilities
- Writes clinical program objectives, policies, procedures and manuals.
- Conducts individual health assessments.
- Develops and reviews individual health care plans.
- Participates on interdisciplinary teams or chairs them.
- Keeps records of individual progress and outcomes.
- Maintains liaison between facility and community programs.
