Job summary

PAM Health Specialty and Rehabilitation Hospitals of Miamisburg is seeking a Clinical Navigator to be the face of our hospital. You will build relationships with referral sources, meet patients and families, and collaborate with the admissions team to streamline referrals.

The role requires a health-related degree and current licensure in a clinical field, with at least two years of relevant experience; marketing or rehab/LTACH experience is preferred.

Qualifications

• Education and Training: Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Experience: Minimum of two years’ experience in the designated field of license; rehabilitation/LTACH and marketing experience preferred.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and conducts sales contacts; analyzes referral and admission data.
• Collaborates with Admissions Manager, admissions staff, nursing and external teams to facilitate referrals and improve customer satisfaction.
• Develops and maintains relationships with stakeholders including patients, families, physicians, discharge planners, case managers and payer representatives.
