You will build relationships with case managers, discharge planners, physicians, and ICU teams, assess patient fit for LTACH, educate stakeholders on program benefits, coordinate admissions, and track outcomes to drive performance bonuses.

Full-time | Monday–Friday | Salary + performance bonus | Covers the Columbia, Missouri market

Landmark Hospitals is looking for a Clinical Liaison to grow referrals for Landmark Hospital of Columbia. You’ll be the trusted link between referring hospitals and our team. You’ll make sure medically complex patients, such as those weaning from a ventilator or needing complex wound care or long‑term IV therapy, get the specialized care they need after leaving the ICU.

What you’ll do

• Build and grow referral relationships with case managers, discharge planners, physicians, intensivists, and ICU teams at hospitals across your market.
• Be the LTACH expert in the room. Review patient records, help assess whether a patient is a fit for long‑term acute care (LTACH), and clearly explain next steps to referral sources, patients, and families.
• Educate the market on LTACH benefits, Landmark’s programs, and our outcomes through presentations, in‑services, and one‑on‑one meetings.
• Find new opportunities by researching market data, targeting new referral sources, and building a territory plan with your leadership.
• Coordinate admissions with the hospital team so referrals turn into smooth, timely admissions.
• Track your activity and results in our referral system and report on progress toward goals.

What success looks like

• In your first 90 days, you know the key decision‑makers at your top referring hospitals by name.
• Referral sources call you first when they have a complex patient.
• Referrals, admissions and conversion rates grow month over month, and your bonus reflects it.

What you bring

Required

• At least 1–3 years of experience in healthcare, healthcare sales, or business development
• Strong relationship‑building, communication, and presentation skills
• Self‑motivated and organized, able to manage your own schedule and territory
• Valid driver’s license, reliable transportation, and the ability to travel by car throughout the market

Preferred

• Active, unrestricted clinical license: RN, LPN, RT, PT, PTA, OT, COTA, or SLP
• Experience in LTACH, acute care, ICU, case management, or post‑acute referrals
• Associate or bachelor’s degree. Equivalent experience will be considered.

• Competitive base salary of $80K - 90K plus a performance-based bonus program
