Human Touch Home Health in Richmond, VA seeks a Registered Nurse (RN) Clinical Manager to lead the clinical team on-site. This full-time leadership role requires hands-on oversight to ensure safe, compliant, high-quality home health services with direct supervision of RNs, LPNs, and clinical staff.
The ideal candidate has 3+ years of nursing management in home health, strong OASIS knowledge, and a track record of improving clinical quality.
Qualifications
- Active Virginia RN license or multi-state license valid in Virginia.
- Minimum 3 years nursing management or clinical leadership experience in Home Health or related setting.
- Strong knowledge of skilled home health operations and documentation.
- Onsite 100% presence in Richmond, VA.
- Onsite schedule Monday–Friday, 8:30 AM–5:00 PM.
Responsibilities
- Provide daily leadership and supervision of the clinical team.
- Support and manage RNs, LPNs, therapists, and other clinical staff.
- Monitor staff performance, productivity, and patient-care responsibilities.
- Review clinical documentation for accuracy, quality, and compliance.
- Provide oversight of OASIS documentation and requirements.
- Review Start of Care, Resumption of Care, Recertification and Discharge documentation.
- Monitor patient plans of care and ensure physician orders are followed.
- Assist with case conferences and interdisciplinary care coordination.

Human Touch Home Health is seeking an experienced Registered Nurse (RN) Clinical Manager to provide hands-on clinical leadership for our Richmond, Virginia office.
We are looking for a nurse leader who understands home health, can effectively manage a clinical team, knows the importance of compliance and quality documentation, and can balance accountability with support.
This is a full-time, in-office leadership position.
Candidates must meet the following minimum requirements:
- Active Registered Nurse (RN) license in Virginia OR an active multistate/Compact RN license that permits practice in Virginia
- Minimum 3 years of nursing management or clinical leadership experience in Home Health, Home Care, or a closely related healthcare setting
- Strong knowledge of skilled home health clinical operations
- Ability to work 100% onsite in Richmond, Virginia
- Ability to work Monday through Friday, 8:30 AM–5:00 PM

As the Clinical Manager, you will provide daily clinical leadership and oversight while helping ensure patients receive safe, timely, compliant, and high-quality home health services.
Responsibilities include:
- Provide daily leadership and supervision of the clinical team
- Support and manage RNs, LPNs, therapists, and other clinical staff
- Monitor clinical staff performance, productivity, and patient-care responsibilities
- Review clinical documentation for accuracy, completeness, quality, and compliance
- Provide oversight of OASIS documentation and requirements
- Review Start of Care, Resumption of Care, Recertification, and Discharge documentation
- Review and monitor patient plans of care
- Ensure physician orders are obtained, followed, and appropriately documented
- Monitor changes in patient condition and ensure appropriate clinical follow-up
- Assist with case conferences and interdisciplinary care coordination
- Monitor patient census and clinical staffing needs
- Help ensure patients are assigned to appropriate clinicians and visits are completed timely
- Address missed visits, delayed care, and other clinical concerns
- Provide clinical guidance and support to field staff
- Coach, mentor, educate, and hold clinical employees accountable
- Participate in clinical orientation and ongoing staff education
- Assist with competency evaluations and performance management
- Support quality assurance and performance improvement initiatives
- Help maintain survey readiness and regulatory compliance
- Work closely with the Administrator, Intake, Staffing, Recruitment, and other departments
- Utilize WellSky for clinical oversight, documentation review, and workflow management

- Knowledge of OASIS and Medicare home health requirements

- Demonstrated leadership and performance-management skills
- Excellent verbal and written communication
- Strong documentation and compliance knowledge
- Excellent organizational and time-management skills
- Ability to manage multiple clinical priorities in a fast-paced environment
- WellSky experience strongly preferred
This is not a position where the Clinical Manager leads the team from home.

The 8:30 AM–5:00 PM schedule is required.
Daily onsite presence in the Richmond office is required.
Remote work, hybrid work, and alternate schedules are not available for this position.
