This is an excellent opportunity for an experienced RN Supervisor or nursing professional ready to take the next step into management. The role will initially involve hands‑on patient care and clinical operations, with increasing responsibility for team leadership, staff development, and nursing oversight as the program grows.

PACE (Program of All-Inclusive Care for the Elderly) experience is preferred but not required.

Key Responsibilities
Clinical Care and Coordination
  • Provide direct nursing care to participants in the clinic.
  • Develop and maintain individualized care plans and support ongoing participant assessments.
  • Collaborate closely with the Primary Care Physician, Nurse Practitioner, and interdisciplinary care team to coordinate participant care.
  • Support the management of complex clinical situations and promote continuity of care.
  • Ensure clinical documentation and nursing services meet applicable organizational, state, and federal requirements.
Clinical Operations and Program Development
  • Help establish nursing workflows, clinical protocols, and procedures.
  • Participate in daily interdisciplinary team meetings.
  • Support clinic readiness, including equipment ordering, clinical processes, and operational preparation.
  • Develop proficiency in the electronic medical record (EMR) system and help maintain accurate clinical documentation.
  • Assist with staff training, onboarding, and ongoing clinical education.
Nursing Leadership and Management
  • Supervise, coordinate, and evaluate nursing services.
  • Provide guidance, mentorship, and support to nursing team members.
  • Monitor clinical workflows and promote consistent, high-quality care.
  • Help grow and develop the nursing team as participant census increases.
  • Gradually transition from primarily direct patient care to broader clinical oversight and team management.
Qualifications
  • Active California Registered Nurse (RN) license in good standing.
  • Graduate of an accredited nursing program; Bachelor of Science in Nursing (BSN) preferred.
  • At least three years of nursing experience.
  • At least one year of experience working with frail, elderly, or geriatric populations.
  • At least one year of supervisory experience in an outpatient clinic setting.
  • Strong clinical judgment and the ability to manage complex patient care situations.
  • Excellent organizational, communication, and leadership skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Comfort providing hands‑on patient care while helping develop and expand a clinical program.
Preferred Qualifications
  • Experience in a PACE program or a similar coordinated care setting.
  • Experience developing clinical protocols, care plans, and nursing procedures.
  • Experience training, mentoring, or supervising nursing staff.
  • Familiarity with electronic medical record systems.
  • Spanish-English bilingual skills are a plus.
Schedule and On-Call Requirements
  • Monday through Friday, either 8:00 AM–5:00 PM or 7:30 AM–4:30 PM.
  • Participation in an on-call rotation approximately once every three months, covering five days per rotation.
  • After-hours responsibilities are primarily related to clinical coordination, triage, and supporting care management decisions.

If you are an experienced RN who enjoys both patient care and leadership and is excited to help build a growing clinical program, we encourage you to apply.
