Description

Create a single entry point for all Ambulatory services offered within MSHS, Community referrals and assist with throughput. Performs the following tasks in an efficient and cost-effective manner as assigned in order to respond to treatment needs of patients.

Description

Create a single entry point for all Ambulatory services offered within MSHS, Community referrals and assist with throughput. Assesses urgency of evaluation or treatment request and refer patients to appropriate level of care within our Mount Sinai Health System. Performs the following tasks in an efficient and cost-effective manner as assigned in order to respond to treatment needs of patients.

Responsibilities

  • Collects initial clinical information from potential patients and outside referral sources in order to facilitate new referrals
  • Coordinates administrative and data collection efforts in the Central Access Center Hub
  • Provides direct telephone triage service to patients and families
  • Collection and verification of patient insurance information upon intake
  • Monitors managed care authorization and tracks follow-up visits
  • Notifies manager of any problems regarding authorization. Notifies clinician of number of visits allowed and when patient is approaching completion. Follows up with clinician to ensure proper documentation for managed care authorization
  • Collaborates the maintenance of outpatient treatment reports with clinicians and submits to Insurance company in order to retain timely filing
  • Follows up with patients and referral sources in result of "no show" and ensure outreach of MCT if applicable
  • Assists external inpatient units with discharging of new patients and ensuring appropriate level of care is met '
  • Supervises 1 Data Coordinators/ Social Work Assistant and reviews performance on a regular basis in regards to proper intake procedure, working knowledge of insurance coverage for the hospital, managed care authorizations; and all duties assigned
  • Conducts a minimum of monthly administrative meetings with Data Coordinator/Social Work Assistant staff to identify problems; assist in problem resolution, and facilitate communication
  • Attends System staff meetings
  • Maintains appropriate patient care documentation as defined by hospital policies and procedures
  • Sets appropriate priorities and responds to follow-up situations in acceptable time period
  • Demonstrates ability to make decisions and manages resources
  • Demonstrates sensitivity to patient/family cultural background and spiritual needs
  • Responsible for data entry and maintenance of patient database, excel spreadsheets, Word documents as requested by the supervisor
  • Facilitates the intake process; communicate with registrars regarding insurance information
  • Runs reports and statistical analyses as needed.
  • Coordinates intake dates and times with clinicians
  • Effectively represents the department' s philosophy and policy to other departments and community agents

Qualifications

Education Requirements

  • Master's Degree Required

Experience Requirements

  • Health care experience preferred. May require experience with specific populations. Supervisory experience preferred.

Licensing and Certification Requirements (if applicable)

  • LCSW is preferred

Compensation

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $95450 - $150519.96 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
