The role you’ll contribute:
The Clinical Manager is a Registered Nurse or Physical Therapist who coordinates and supervises the overall clinical care such as scheduling, coordination and supervision of the home health interdisciplinary team. The interdisciplinary team, in consultation with the patient/family and physician(s), collectively assesses, coordinates, develops and implements a plan of care (POC) that is directed towards teaching the patients/families/caregivers how to manage the illness at home, assist the patient and family through the rehabilitative process, and teaches them how to return to the highest level of functioning possible to maintain independence in their own environment. This position may be designated as the Director of Nursing and/or Alternate Administrator for regulatory purposes.
This position supervises and directs the activities of various levels of assigned nursing and ancillary personnel utilizing both professional and supervisory discretion and independent judgment, while practicing within the State, facility and regulatory guidelines.

Supervises staff providing direct patient care such as home health aide, RN, LPN, PT, PTA, OT, COTA, SLP, MSW. May also supervise other office staff who support the delivery of care as per organization chart.
