The role you’ll contribute:

The Clinical Manager is a Registered Nurse responsible for coordinating and supervising the overall clinical care, including scheduling and overseeing the home health interdisciplinary team. The team, in consultation with patients, families, and physicians, assesses, develops, and implements a plan of care (POC) aimed at teaching patients and caregivers how to manage illnesses at home, supporting rehabilitation, and promoting independence. This position may serve as the Director of Nursing and/or Alternate Administrator for regulatory purposes.

This role supervises various nursing and ancillary staff, exercising professional and supervisory judgment within regulatory guidelines.

• Plan, coordinate, and manage all clinical activities for home health in compliance with regulations and standards.
• Provide clinical oversight and scheduling to ensure optimal operations.
• Monitor operational indicators to identify changes impacting staffing, productivity, revenue, quality, and patient satisfaction.
• Implement continuous improvement initiatives to deliver quality, cost-effective patient care.
• Develop patient-specific POCs with clinicians, ensuring completeness and alignment with assessments.
• Collaborate on establishing correct ICD-10 codes with clinicians and coding resources.

Qualifications:

• Associate’s degree in nursing or a related healthcare field.
• At least three (3+) years of experience in home care or a related healthcare setting.
• Active Registered Nurse or Physical Therapy license issued by the state of practice.
• Valid driver’s license, reliable vehicle, and insurance, with a good driving record.
• Basic Life Support certification.

Preferred qualifications:

• At least one (1) year of experience in clinical supervision or quality review.
• Bachelor’s degree in nursing or a related healthcare field.
