Position Summary

The Clinical Manager is responsible for overseeing the clinical operations of the home health aide agency, ensuring the delivery of high-quality, patient-centered care in compliance with all federal, state, and local regulations. This role provides leadership and supervision to nursing and aide staff, monitors patient outcomes, ensures regulatory compliance, and supports the agency’s mission to deliver safe, compassionate care in the home setting.

Key Responsibilities
Clinical Oversight & Supervision
  • Supervise and support nursing staff, home health aides, and other clinical personnel.
  • Conduct supervisory visits and in-home assessments to evaluate the quality of care.
  • Monitor clinical documentation for accuracy, timeliness, and compliance with regulatory requirements.
Patient Care Management
  • Coordinate initial and ongoing patient assessments and develop individualized care plans.
  • Ensure appropriate assignment of staff based on patient acuity, licensure, and skill level.
  • Review and evaluate patient outcomes, making adjustments to care plans as necessary.
Regulatory Compliance & Quality Assurance
  • Maintain compliance with Medicare/Medicaid regulations, HIPAA, and DC Department of Health standards.
  • Participate in internal audits, chart reviews, and quality improvement initiatives.
  • Respond to complaints, incident reports, and other quality concerns in a timely, professional manner.
Leadership & Team Development
  • Provide coaching, training, and performance evaluations for clinical staff.
  • Assist in hiring, orientation, and ongoing development of nursing and aide personnel.
  • Foster a culture of accountability, teamwork, and excellence in patient care.
Administrative & Operational Support
  • Collaborate with the Administrator, Office Manager, and Staffing Coordinator to ensure efficient service delivery.
  • Participate in agency meetings, case conferences, and interdisciplinary care planning.
  • Maintain up-to-date knowledge of industry best practices and clinical guidelines.
Qualifications
Education & Licensure
  • Registered Nurse (RN) license in good standing with the District of Columbia.
  • Bachelor of Science in Nursing (BSN) preferred.
  • CPR certification required; additional certifications in home care or case management a plus.
Experience
  • Minimum 3–5 years of clinical experience in home health, hospice, or community-based care.
  • At least 1–2 years of supervisory or leadership experience preferred.
  • Familiarity with Medicare Conditions of Participation and home health standards.
Skills
