Job summary

AlphaCare Home Health Corp. in San Francisco, CA is seeking an experienced RN Case Manager Supervisor to lead clinical services, ensure CMS compliance, and coordinate care for geriatric patients in home health.

You will oversee recruitment, training, and supervision of nursing and rehab staff, ensuring high-quality care and adherence to regulations. You will also review OASIS documentation, supervise field staff, and drive quality improvement while maintaining regulatory standards.

Qualifications

  • Current and unrestricted California Registered Nurse license.
  • Minimum of 1 year of experience as a Registered Nurse Case Manager in home health.
  • Knowledge and expertise in clinical decision-making for geriatric patients in home health.
  • Ability to provide care to geriatric populations, with pediatric/adult as needed.
  • Professional demeanor and appearance.
  • Excellent oral and written skills.
  • Leadership or precepting experience preferred.
  • Valid driver’s license and reliable transportation.

Responsibilities

  • Assist with recruiting, hiring, and ongoing in-service education of all clinical staff.
  • Supervise nursing and rehab clinical activities and staff.
  • Provide guidance and support for supervisory, field, and office staff.
  • Cover field nurses to visit patients during staffing shortages.
  • Train new clinicians in clinical skills and documentation systems.
  • Accountable for all clinical issues.
  • Ensure coordination of care by all disciplines.
  • Ensure CMS documentation compliance; review OASIS and other documents.
  • Reassess patient needs as health status changes.
  • Investigate and resolve patient care complaints as needed.
  • Ensure care delivery meets professional standards.
  • Develop, implement, and revise care delivery policies and procedures.
  • Conduct employee performance evaluations and update job standards.
  • Counsel staff on deficiencies and areas for improvement.
  • Ensure compliance with laws, policies, and plan of care.
  • Stay informed on home health regulations and management issues.
  • Ensure compliance with The Conditions of Participation.
  • Participate in program evaluation and growth planning.
  • Increase market share through physician and referral education.
  • Engage with professional organizations and meetings.
  • Perform other related duties.
  • Able to lift 50 lbs.

Job description

Assume primary responsibility for clinical services provided by the Agency. Direct, coordinate and supervise all clinical functions. Maintains compliance with federal and state regulations as they apply to the provision of home visits to patients. Assure compliance with Agency Policy and Procedure. Monitor job performance and completion of evaluations and counseling of clinical staff.

Qualifications

  • Have sufficient background knowledge and expertise in clinical decision-making for the patient population of the home health agency to meet the needs of his or her patients, and to contribute to Quality Management review and evaluation.
  • Demonstrate knowledge and skills necessary to provide care primarily to the geriatric population, and to a lesser degree, the pediatric and adult population.
  • Professional demeanor and appearance.

  • Cover field nurses to visit patients as needed during staffing shortages.
  • Provide training to new clinicians including clinical skills and electronic systems used for documentation and communication.
  • Accountable for all clinical issues.
  • Ensure coordination of care by all disciplines.
  • Ensure all documentations are compliance with CMS guidelines. Review and approve OASIS and other documentations by nurses and other clinicians.
  • Assure that a reassessment of a patient’s needs is performed by the appropriate health care professional (i.e. when there is a significant health status change in the patient’s condition at the physician’s request or after hospital discharge.)
  • Assure service satisfaction through client visits and other measures, as appropriate. Investigate and resolve agency complaints related to patient care if required.
  • Ensure care delivery meets the needs of the patients and follows professional practice standards.
  • Develop, implement, review and revise policies and procedures to guide care delivery.
  • Conduct employee performance evaluations. Assists with development of new job standards as needed.
  • Counsel staff appropriately and in a timely manner concerning areas of deficiency as well as areas of excellence and quality improvement.
  • Assure compliance with company policy and all federal, state, and local regulatory bodies.
  • Monitor compliance with applicable Federal, State and Local laws, Professional Standards and principles, established policies and procedures and the plan of care.
  • Remain informed of home health regulations and standards as well as management issues.
  • Ensure compliance with The Conditions of Participation.
  • Participate in review, analysis and appraisal of the effectiveness of the agency programs.
  • Participate in short and long range planning for the agency and implement specific measures for agency growth.
  • Increase market share through education of physicians and other community and referral sources.
  • Participate in state, local and national organizations, meetings, seminars, workshops and activities relating to the home health profession and health care services.
  • Perform other related and assigned duties.
  • Able to lift 50lbs.

Compensation: $135,200-$156,000 per year
