Immediate Need: Clinical - Senior LTSS Service Care Manager (RN)

This is a 6+ months contract opportunity with long-term potential, located onsite in Hidalgo and Brownsville, TX. Please review the job description below and contact us ASAP if interested.

Pay Range: $40 - $45/hour. Benefits include health insurance (medical, dental, vision), 401(k), and paid sick leave (depending on work location).

Key Responsibilities:

• Completing assessments
• Conducting biannual reviews
• Care coordination
• Following up on requests from members / LAR
• Completing assigned trainings
• Documentation and authorization submissions
• Travel for home assessments
• Maintaining at least 92% audit score
• Member / LAR education
• Preparing productivity reports
• Following up on HEDIS measures
• Assisting members / LARs with healthcare provider arrangements

Key Requirements and Technology Experience:

• Experience in Pediatrics, Home Health, RN, and Pediatrics preferred
• RN license required; Pediatrics experience is a plus
• Proficiency in Outlook, Excel, Word, Microsoft Teams
• Graduate from an accredited nursing school or hold a bachelor’s degree with 4–6 years of related experience
• Bachelor’s degree in nursing preferred
