Job summary

Signature HealthCARE KY, Lexington is seeking LPNs to provide direct nursing care to residents across our Lexington-area facilities, including Bluegrass Care & Rehab and Mayfair Manor. You will administer medications, assist with ADLs, supervise nursing assistants, monitor residents, and contribute to care planning and documentation, ensuring quality, safety, and compassionate service.

Qualifications

• Active LPN/LVN license in Kentucky.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in supervisory or charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents' physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
