Job summary

Signature HealthCARE is seeking motivated LPNs to provide direct nursing care across skilled nursing and rehab settings. Full-time and part-time opportunities are available at Lexington-area facilities, including Bluegrass Care & Rehab and Mayfair Manor.

Responsibilities include medication administration, ADL assistance, supervising nursing staff, maintaining care plans, and accurate documentation. A current LPN/LVN license, US-based practice, and CPR/BLS are preferred; prior long-term care

Qualifications

• Current, active LPN/LVN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Strong clinical observation, medication administration, and documentation skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents' physical and emotional status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
