SNF Healthcare is seeking a personable clinical liaison to build referral networks with hospitals, discharge planners, and community partners. You will conduct pre-admission assessments and foster relationships to support residents and their families.

The role involves weekend coverage and a flexible schedule, with 3 days per week and every weekend. Strong communication and problem-solving abilities are essential for success in this role.

• Ability to connect with healthcare professionals, potential clients, family members and community leaders.
• Resourceful problem solver.
• Flexible, self-driven, accountable, autonomous yet team-oriented.
• Preference for 1 year of long term care clinical liaison experience.
