Summary

The Nurse Liaison ensures that the palliative medicine needs of patients are met. While primarily patient-focused, the role also involves community education and customer service. The Nurse Liaison develops strong relationships within the community to enhance the image and acceptance of the palliative medicine team and VIA Health Partners. The role collaborates closely with team members, including the Strategic Accounts Manager and AVP, to ensure consistent communication and high-quality service. When appropriate, the Nurse Liaison assists the admission team to facilitate smooth transitions to hospice care.

Essential Functions
  • Review medical records for new Palliative Medicine referrals; manage patient contacts based on acuity; document interactions; serve as the main contact for patients, families, and healthcare providers; facilitate seamless hospice transitions; collaborate on scheduling; educate patients and families; monitor intervention efficacy; manage patient data; coordinate workload with team members.
  • Coordinate community education efforts with the Strategic Account Manager; provide impromptu educational sessions on palliative care topics.
  • Maintain collaborative relationships with administrative and community staff; support business development; assist with service recovery; communicate community updates to leadership; participate in community events.
  • Assist in policy development; educate staff on palliative medicine topics; perform additional duties as needed.
Qualifications

Minimum Qualifications

  • Diploma or associate degree in nursing required; Bachelor’s degree preferred.
  • Current, active, and unrestricted multistate license as RN or LPN required.
  • CPR certification required; CHPN preferred.
  • At least 2 years of relevant experience, including direct care of patients with life-limiting illnesses; experience in hospice, palliative care, or related fields preferred.
  • Ability to perform nursing assessments required.
