The Care Team Hospice is seeking a Hospice Clinical Liaison (RN/LPN) to strengthen partnerships with hospitals, skilled nursing facilities, and physicians. You will educate providers, assess hospice eligibility, and ensure smooth transitions from referral to admission. Pleasant Area and be driving an hour in any directionThe Care Team Hospice is seeking an experienced and relationship-focused Hospice Clinical Liaison (RN/LPN) to help strengthen our partnerships with hospitals, skilled nursing facilities, senior living communities, physicians, and other healthcare providers. This is a unique opportunity for a clinician who enjoys being out in the community, building relationships, educating others, and connecting patients with the right care at the right time.As a Hospice Clinical Liaison, you'll serve as the bridge between our clinical and sales teams, bringing your healthcare expertise directly to our referral partners.
