Job summary

Overview

At Central Texas Rehabilitation Hospital, you will serve as a Clinical Liaison, supporting community health through patient referral and assessment to appropriate inpatient rehabilitation services. Your role enables patients to access the right care and facilitates collaboration between referring facilities, patients, and the hospital.

Responsibilities

• Builds and maintains referral relationships to drive appropriate admissions to the hospital
• Assist and coordinate in-take and pre-admission screening process
• Perform on-site clinical judgment to determine appropriateness of patient for admission
• Communicate with patients and their families about rehabilitation and facility options, including expectations and participation requirements
• Secure information relating to patients' resources and benefits
• Functions as initial contact for external case management and discharge planner personnel with payors and hospital facilities

Qualifications

• Clinical licensure as an RN, LVN, PT, PTA, OT, COTA, SLP, RT or MSW and a bachelor's degree
• Previous marketing/sales experience preferred
• Previous clinical experience preferred, with demonstrated skills in clinical assessment and EMR systems
• Formal sales training preferred
• Valid driver's license
