Job summary

Wind Crest by Erickson Senior Living in Highlands Ranch, CO is seeking a Clinical Liaison to manage data collection, referrals screening, and admissions coordination for our Continuing Care Retirement Community.

You will collaborate with physicians, discharge planners and care teams to determine transfer appropriateness, plan transitions from acute hospital to Continuing Care or Independent Living, and support resident moves while complying with state regulations.

Qualifications

• CCRC, Skilled Nursing, Memory Care / Assisted Living experience highly preferred.
• Liaison experience or healthcare marketing background preferred.
• Familiarity with acute hospitals, rehabilitation hospitals, sub-acute rehabilitation facilities, and the local healthcare market preferred.

Responsibilities

• Perform onsite clinical collection of data of the referrals to determine appropriateness for transfer to Continuing Care.
• Perform discharge planning and care coordination for residents transferring from acute hospital to Continuing Care or returning to Independent Living at the Erickson Living community.
• Collaborate with physicians regarding anticipated discharges from facilities.
• Identify quality issues and participates in quality management activities, as needed, with Administrator and/or Medical Director.
