• RN or RT license required; other designations considered.
• Ability to complete clinical assessments of medically complex patients.

Responsibilities

• Plan the best continuum of care after ICU or post-surgical stay with physicians, discharge planners and families.
• Counsel patients' families to assist in making the best care decisions for loved ones.
• Collaborate with case managers and physicians to smooth pre-admission and transfer processes, removing payer-related and operational barriers.
