Job summary

Luxor Healthcare is seeking an energetic Clinical Liaison to support an expanding network of skilled nursing and rehabilitation communities across the Kansas City area. The role emphasizes building relationships, coordinating patient transitions, and connecting individuals with high-quality post-acute care services.

You will collaborate with hospitals, physicians, case managers, and community partners to drive admissions and occupancy growth while promoting a positive facility image within the

Qualifications

• 2+ years of admissions, business development, clinical liaison, or related healthcare experience.
• Experience in skilled nursing, rehabilitation, post-acute care, or hospital settings preferred.
• Strong communication, organizational, and relationship-building skills.
• Professional customer service and phone skills.
• Knowledge of Managed Care processes preferred.
• LPN license preferred but not required.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, case managers, and community organizations.
• Review referrals and coordinate admissions processes, including assessments, documentation, and authorizations.
• Conduct facility presentations and tours for referral sources and families.
• Communicate resident needs to facility teams to ensure smooth transitions.
• Promote a positive image of our facilities within the community.
• Maintain knowledge of Medicare, Medicaid, and Managed Care processes.
• Travel between facilities and throughout the Kansas City market to support referral development and admissions growth.
