In this regional role you will coordinate admissions, evaluate referrals, and ensure smooth post-acute transitions, traveling between sites to build trust with healthcare partners and residents.

Starting salary is $110K/year with mileage reimbursement and private health care; opportunities for professional

Qualifications

• Active LPN or RN license (preferred) or clinical background in healthcare, case management, or marketing.
• Experience in skilled nursing, post‑acute, or hospital‑based discharge planning preferred.
• Excellent communication, organization, and relationship‑building skills.
• Ability to work independently and travel regularly between facilities.
• Must reside within commuting distance of Tacoma, WA

Responsibilities

• Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers.
• Evaluate patient referrals and assist with clinical screenings and pre‑admission assessments.
• Coordinate admissions and ensure timely, efficient transitions from acute to post‑acute care.
• Serve as a clinical resource and liaison to families and referral sources.
• Collaborate with internal facility teams to align care plans and meet resident needs.
• Track referral activity and participate in census development initiatives

In this regional role, you’ll serve as the key connection between hospitals, families, and our facilities — helping patients transition to post-acute care with confidence and clarity.
