Evergreen Healthcare Group in Seattle is seeking a Clinical Liaison to bridge hospitals, families, and post-acute care teams. In this regional role, you’ll coordinate admissions, evaluate referrals, and support smooth transitions while delivering compassionate, relationship-driven care.

The role requires active LPN or RN license (preferred) or equivalent clinical background; travel between facilities; and residency within commuting distance of Seattle.

Qualifications

• Active LPN or RN license or clinical healthcare background.
• Experience in skilled nursing, post-acute, or hospital discharge planning preferred.
• Excellent communication, organization, and relationship-building skills.
• Ability to travel regularly between facilities.
• Must reside within commuting distance of Seattle, WA.

Responsibilities

• Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers.
• Evaluate patient referrals and assist with clinical screenings and pre-admission assessments.
• Coordinate admissions and ensure timely, efficient transitions from acute to post-acute care.
• Serve as a clinical resource and liaison to families and referral sources.
• Collaborate with internal facility teams to align care plans and meet resident needs.
• Track referral activity and participate in census development initiatives.

You’ll play a critical role in coordinating seamless admissions, building trust with healthcare partners, and ensuring patients receive the care they need in a setting that supports healing and quality of life.
