Evergreen Healthcare Group is seeking a Clinical Liaison to serve skilled nursing facilities in the Portland/Hillsboro market. The role connects hospitals, families, and our facilities to ensure smooth transitions to post-acute care with compassion and clarity.

The position requires active LPN or RN credentials (preferred) or other clinical background, with opportunity for professional development and travel between facilities within the Portland/Hillsboro area.

Qualifications

• Active LPN or RN license (preferred) or clinical background in healthcare, case management, or marketing
• Experience in skilled nursing, post-acute, or hospital-based discharge planning preferred
• Excellent communication, organization, and relationship-building skills
• Ability to work independently and travel regularly between facilities
• Must reside within commuting distance of Portland/Hillsboro, Oregon

Responsibilities

• Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers
• Evaluate patient referrals and assist with clinical screenings and pre-admission assessments
• Coordinate admissions and ensure timely, efficient transitions from acute to post-acute care
• Serve as a clinical resource and liaison to families and referral sources
• Collaborate with internal facility teams to align care plans and meet resident needs
• Track referral activity and participate in census development initiatives
