Job summary

Select Specialty Hospital in Grand Rapids seeks a Clinical Liaison (RN / RT) to advocate for referred patients, coordinate post-ICU care, and collaborate with clinical teams. You will foster relationships with physicians and families, supporting efficient pre-admissions and transfers to our LTACH settings.

Qualified candidates hold RN or RT licensure, with flexibility for other designations; strong communication and problem-solving skills are essential for success in this role.

Qualifications

• Registered Nurse (RN) or Respiratory Therapist (RT) license required.
• Other designations like LPN, PT, OT may be considered.
• Ability to complete clinical assessments of medically complex patients.

Responsibilities

• Work with physicians, discharge planners and families to plan the continuum of care after an ICU or post-surgical stay by utilizing your strong clinical background, payer knowledge and problem-solving skills.
• Perform clinical assessments of referred patients, then counsel with patient's families to assist them in making the best care decisions for their loved one.
• Partner with hospital case managers and physicians to create a smooth pre-admissions and transfer processes through removing payer-related and operational barriers for the patients getting into our highly effective level of care.

Critical Illness Recovery Hospital (LTACH)

Annual salary ranges from $70,000 to $93,000 based on relevant work experience.

Uncapped Monthly Bonus Incentives in addition to base salary.

A day in the life

You’ll advocate for referred patients in the pre-admission process by collaborating with clinical teams to assess patient needs. You will serve as a resource for healthcare professionals through fostering relationships within medical facilities. With clinical expertise and problem-solving skills, you’ll also coordinate care plans with physicians and families. Additionally, you will facilitate patient transfers and address barriers to accessing specialized post-ICU care.

• Full-time: Monday-Friday 8-5pm,

What we're looking for in a Clinical Liaison

Required Skills:

• Registered Nurse (RN) or Respiratory Therapist (RT) License.

• Will consider other designations like LPN, PT, OT etc., upon further review.
• Ability to complete clinical assessments of medically complex patients.

Preferred skills that will make you a successful Clinical Liaison:

• Demonstrated success in patient advocacy.
• Superior communication and relationship-building skills.
• Prior healthcare marketing or business development experience is preferred, but extensive training is provided on this piece.
